Questions and Answers

Find expert answers from podiatrist Dr. Milt Stern to the most frequently asked questions about custom orthotics and foot biomechanics.

1. What do orthotics do?

There are three primary functions of custom orthotics:

  • Control Motion: If a foot exhibits excessive pronation (flattening), the orthotic is engineered to stabilize and restrict harmful motion.
  • Absorb Shock: High-impact forces during ground strike are cushioned by specialized top covers, reducing joint strain.
  • Accommodate Painful Conditions: Specific areas of foot pain or callus buildup are offloaded using targeted padding tailored to your anatomy.

2. Do I need custom orthotics instead of store-bought inserts?

Custom orthotics are individually crafted from a 3D impression of your feet to address specific anatomical conditions like overpronation, flat feet, or rigid high arches. Over-the-counter (OTC) arch supports are mass-produced with flexible plastics that often collapse completely under weight. Custom orthotics provide rigid structural control and last significantly longer than store-bought alternatives.

3. How long do I have to wear orthotics?

While some orthotics are prescribed for short-term rehabilitation (such as taking pressure off an inflamed posterior tibial tendon to let it heal), continuous long-term use is recommended. Wearing custom orthotics consistently helps maintain optimal biomechanical alignment and prevents recurring overuse injuries.

4. Will orthotics fit in all of my shoes?

Orthotics are crafted in several shapes and profile lengths. Full-length orthotics fit athletic sneakers, boots, and dress shoes with removable insoles, while 3/4-length orthotics fit low-profile shoes like loafers or dress flats. During your evaluation, we select the correct profile for your primary footwear. We also offer discounted rates for additional pairs so you can have specialized designs for both dress shoes and athletic footwear.

5. How often do I have to replace custom orthotics?

Orthotics4U builds shells with deep heel cups and Schaffer plates (extended support shelves) for high structural durability. Because our solid shell structures outlast typical competitor designs, you don't need to replace the entire orthotic when padding wears down. We offer refurbishment services at approximately one-third the cost of a new pair, returning your orthotics to like-new condition.

6. How can custom orthotics help relieve back pain?

Excessive foot pronation causes internal rotation of the ankle, which forces inward rotation of the knee, leg, and hip. This kinetic misalignment frequently leads to lower back pain. By controlling foot pronation at the foundation, orthotics restore natural alignment throughout the legs and spinal column.

7. Can orthotics prevent further injury or discomfort?

While no device guarantees complete injury prevention, custom orthotics are proven tools for injury management. They provide joint stability, shock absorption, and alignment control essential for managing conditions like plantar fasciitis, heel spurs, and Achilles tendonitis.
